If your child is 14 and older, they will need to sign the paperwork with you.

  • Informed Consent For Treatment By signing this document you are allowing Catalpa Health to provide treatment for your child.
  • Notice of Privacy Practices By signing this document you acknowledge that Catalpa Health has offered you a copy of the privacy practices (above) and that Catalpa follows HIPAA laws and will protect your healthcare information. 
  • Financial Agreement By signing this document you allow Catalpa Health to bill your insurance company for services rendered, and that you are financially responsible for any balance not covered by your insurance benefits.

Our call center hours are 8:00 to 4:30 Monday – Friday.

 The first scheduled appointment (initial evaluation) serves to determine what services we can provide to help your child. Your Intake Specialist will gather information from the referral source, your family, and use their clinical knowledge to help create a care plan that is right for you. Sometimes based on the care plan or our organization’s availability, the client may be referred outside of Catalpa to partner organizations. If you are coming to us for medication management services, please note that medications will not be prescribed at this first visit, which is an assessment only appointment.

The paperwork can only be signed by a biological or adoptive parent. If the child is being cared for by anyone by a biological or adoptive parent, please call us. We know that step-parents, grandparents, foster parents and other family members play an important role in a child’s life, however, these individuals are not able to legally consent to mental health services in the state of Wisconsin.

Please note: We are in a time of high need for mental health therapy services. Catalpa, and other similar agencies within our community, are scheduling 4-6 weeks out for therapy services and using waitlists to help capture the demand.  At the end of your intake appointment, the counselor will work to connect you with services.  Our first priority will be to get you connected with a therapist within 4-6 weeks, either within or outside of our agency, and a waitlist may be used on a limited basis when no other services are available.  If you are placed on a waitlist, you will have the option of a brief therapeutic check in every few weeks until we can get you started or you can use our Urgent Clinic services should a crisis arise or you need additional support.  We are able to still connect families in a timely way with psychiatry services and help people get started with the psychological testing process, if those are things that you need. If you are interested in psychological testing or an ADHD evaluation for your child, please know that these specialty services are also in high demand. The initial appointment will be an opportunity to share your concerns with an intake specialist to see whether you child would qualify for these services. If so, a referral will be made and your child will be put on a waitlist.
